* Add IOptionsMonitor<IInternalConfiguration>

* Activate change token from *ConfigurationRepository instead of FileAndInternalConfigurationSetter; add acceptance & integration tests

* Update documentation

* Use IWebHostEnvironment as IHostingEnvironment deprecated
